2018 CHILDCARE REQUEST FORM

 
We will offer childcare for the following:
  • Evening and afternoon services - children ages 18 months to 7 years old
  • Morning Services - ages 18 months through Pre-K
In order for us to provide childcare we need you to register in advance.  The cost will be based upon the number of children we receive reservations for.  
 
REGISTRATION DEADLINE IS SEPTEMBER 4, 2018!
